Check out Eric Hamiter's Creating Firefox extensions for a great introduction to building Firefox extensions, in a nice tutorial format. Link via asymptomatic.net.

Unfortunately, there's no "How do I get ideas for new highly useful extensions?" :-)

For inspiration check out the extensions forum. There are always users looking for an extension that does something that has not been written. Occasionly people ask on irc as well.

Inspiration - I want a firefox auto-scrolling screen grab utility built in that saves pngs of websites with the domain as the filename - ie mozilla.org[01-09-04].png - one click does it all with out interupting my browsing.
One was being worked on but is now dead :( - this to me is pretty much the last extension I need, so if you want a good idea - there's one for you.
:)

that scrolling-screenshot-extension would rock my world as well, so chalk up another one for the suggestion! :^)
